These are the stages that make up the second track programme. The sequence is set, the content varies per employee, and the Gatekeeper Improvement Act governs how it is documented.
Intake meeting and situation overview
This step takes one long conversation and sometimes a second. Capacity, work history and wishes come onto the table, and with healthcare work we separate straight away what is wrong with the content from what is wrong with the roster. An action plan emerges that stays within the stated requirements.
Capacity analysis & occupational health assessment
How long this step takes depends on how complete the medical papers are. The FML and the other information determine which work remains, with lifting, standing and irregular hours weighed separately. The result enters the reintegration plan as an objective, with monthly notices to all parties involved.
Career scan & personal coaching
This step runs over several weeks, because self-insight cannot be rushed. Tests, assignments and conversations bring talents, drivers and limits to the surface, and produce a search profile the employee can take to an unfamiliar employer.
Labour market exploration & job matching
This step is short where the direction is clear and longer where it has yet to emerge. The profile goes alongside the market, with Rotterdam, The Hague and Utrecht counting as long as the travel time holds. Should one leap prove too much, a practical period precedes it.
Active job search & network approach
This step continues until something has been found. The employee approaches employers deliberately and through their network, including where nothing is advertised, with CV, LinkedIn and interview preparation alongside. Each week the coach and employee discuss what was undertaken and what it produced.
Aftercare and sustainable placement
This step lasts two months, counted from the first working day. For that period the coach stays available for whatever comes up in the new place, and in the same period the reports and the closing reintegration report are delivered as the UWV asks.

Which demands are still responsible?
In healthcare, lifting, standing and irregular hours occur together, and those three rarely fall away at the same time. The occupational health assessment separates them, which often leaves more open than expected.
Is anything possible within the organisation itself?
At the hospital and the care institutions in this area there are more roles than any single manager oversees. Go through them and keep track of which were examined and for what reason they were ruled out.
How much travel time still fits?
Half an hour to Rotterdam, The Hague or Utrecht triples the supply, but after long-term absence it counts as a load in itself. We agree that limit up front, so it is not quietly stretched later.
Where does capacity get rebuilt?
At a workplace rather than at home. A period in a work experience placement makes clear what holds up in practice and counts as demonstrable effort.

When should we start?
Week 52 is the outer moment; once the company doctor finds a return unlikely, it can be sooner. Starting early counts double here, because trying out a longer journey or a different roster takes time. What the phases cost in lead time is at the duration of a second track programme.
How much does second-track reintegration cost?
That follows from the duration and from the components deployed. Placement support on its own stays limited in scope; adding an occupational health assessment, coaching and a practical period makes it more extensive. You see the composition before anything is set in motion.
Our employee can no longer work irregular hours. What then?
The first question is then whether the work itself is still possible. In healthcare there are roles with the same content but without nights or rotating duties, for instance in outpatient care or day treatment. Make sure that difference reaches the occupational health assessment; it governs how many roles are left.
