What the programme looks like
From intake to aftercare, step by step
At Care4Careers a track 2 programme follows the steps below. What the employee does along the way differs from person to person, but the order and the record-keeping follow the Gatekeeper Act, so the UWV finds nothing to object to later.
Intake meeting and situation overview
The intake conversation puts three things side by side: the medical room, the work history and the direction the employee has in mind. Out of that rolls the action plan, fitting both the UWV's frame and what is practically feasible.
Capacity analysis & occupational health assessment
What is feasible follows from the FML, the remaining medical documents and the coach's judgement on them. Out of that comes a reintegration plan whose goals can be tested, and a monthly update for employer, company doctor and UWV.
Career scan & personal coaching
Conversations, career tests and assignments bring the employee into view. Between them they show the possibilities, the drivers and the limits, which ends in a search profile usable outside the employee's own company as well.
Labour market exploration & job matching
The direction follows from three questions: which work suits the capacity, where that work exists and whether the step can be made in one go. Where there is doubt, a reintegration or work experience placement is slotted in between. Logistics, engineering and healthcare offer the most room here, and that order decides which roles come up first.
Active job search & network approach
Three routes lead to an employer: a deliberate selection, the employee's own network and a direct approach. CV, LinkedIn and interview preparation support all three, and each week the coach goes through what it produced. With Breda and Eindhoven both half an hour's drive away, a considerably larger number of employers falls within reach.
Aftercare and sustainable placement
Several things carry on once the placement is made. The sessions with the coach run another two months, while the reports and the closing reintegration report go to the UWV in the form it requires.

When do we have to start?
Week 52 is the outer limit. Once the company doctor calls a return to the old role unlikely, you start sooner. The duration is set out at the length of track 2. Postponement is the most common reason a file turns out shaky later on.
What are the costs of a second-track programme?
A programme starts at 2,300 euros excluding VAT. The final amount follows from the duration and from what you take: placement on its own is the cheapest form, assessment plus coaching plus a work placement the dearest. The quote comes first and the introductory conversation is free.
Our employee works shifts. Does that change anything?
Yes, and more often in your favour than you would think. Often the roster is the only thing that falls away while the work itself remains perfectly possible. That distinction belongs in the occupational health judgement, because it decides what counts as suitable work.
